The Information Center provides Care Management Services to help locate and arrange care for older persons in their home with the goal of keeping people in their own homes, and out of nursing homes.
The Information Center provides Caregiver Education and Support Groups. People can learn about the resources available to them and their loved one. It is also a supportive atmosphere where people can talk about what has and has not worked for them, what challenges they face, and share their emotions.
The Information Center is the intake site for two prescription drug assistance programs.
The Information Center offers Nursing Facility Transition Services for Medicaid recipients who are living in a nursing home and wish to move back into the community.
