Geriatric Assessment

The purpose of a geriatric assessment is to assess an older adult’s physical and mental status including problems faced by the individual and their family. 

A geriatric assessment is completed by a team of people including a physician, social worker, and nurse.  It may also include a psychiatrist, dietician, or pharmacist. 

Recommendations will be made regarding a treatment plan, educational information, referrals to community resources, and other supportive services needed.
